Youth Nerf Battle Night February 8th 2024

I understand that my teen(s) is participating in a Nerf battle with the Port Maitland Wesleyan Youth group being held at the Port Maitland Consolidated Elementary School gym. As with any physical activity, there is risk of injury. I fully accept this risk and hold harmless from any legal liability, Port Maitland Wesleyan Church and any persons involved in the Youth Group Ministry. In the event of an emergency that requires medical treatment for my teen(s), I understand that every effort will be made to contact me or my emergency contact. In EMERGENCIES requiring IMMEDIATE medical attention, my teen(s) will be taken to Yarmouth Hospital EMERGENCY ROOM. I give my permission for a responsible person from Port Maitland Wesleyan to have my teen(s) transported to that hospital and receive treatment.
